dc.description.abstractAim: Gain insight in which way caregivers who provide care under the Planetree care model, meet the individual needs of older people suffering with dementia on a psycho geriatric ward of a nursing home. Background: The elderly persons suffering with dementia have specific universal needs. They are often unable to explain their individual needs or to have insight into their illness. The Planetree care model is a patient centered, holistic approach to healthcare. Care is provided by ten care components. The individual capabilities and needs of the persons suffering with dementia guide the care. It is unclear how individual care needs of dementia in older people is provided by caregivers who provide care under the Planetree care model. Research in this area is lacking. Methods: This qualitative study was conducted according to the method of Grounded Theory. Twelve purposively selected participants were interviewed using a semi-structured interview. The obtained data were encoded using the method of constant comparison. Results: Participants identify, observe and systematically analyze the behavior of older people suffering with dementia. From these findings interventions were planned to meet the identified care needs. These interventions are then evaluated on results. Understanding the habits, hobbies, character and life of the residents, leads to a more suited intervention to the individual needs of the elderly. Therefore, relatives are actively involved in all stages of the process. Conclusion: The involvement of relatives in care helps caregivers to meet the individual needs of older people suffering with dementia in a better way. Clinical relevance: The acquired knowledge which is displayed by a roadmap could be a supporting strategy for professionals and managers to improve care delivery under the Planetree care model, for the elderly suffering from dementia and live at a psycho-geriatric ward of a nursing home.
