dc.description.abstractHet Prikkelbare Darm Syndroom (PDS) komt naar schatting voor bij 10-20% van de bevolking. Symptomen zijn buikpijn, een verstoord ontlastingspatroon en andere maag-darmklachten, zoals een opgeblazen gevoel, winderigheid en rommelingen in de buik. Tot dusver is er geen somatische oorzaak van PDS geïdentificeerd. Gesuggereerd is dat psychosociale factoren (waaronder catastroferende cognities en stress) een rol spelen bij het in stand houden en verergeren van PDS-symptomen. Symptomen fluctueren gedurende de dag en in de loop van de tijd. Er is echter een gebrek aan inzicht in deze fluctuatie en de samenhang met andere factoren. Elektronische dagboekmonitoring heeft een aantal belangrijke voordelen ten opzichte van schriftelijke vragenlijsten en is geschikt om fluctuerende psychologische en fysieke toestanden te meten in de natuurlijke omgeving op het moment dat ze optreden. Gedurende een aantal weken vulden 13 PDS patiënten drie- of viermaal per dag elektronische dagboeken in. Elektronische dagboekmonitoring bleek voldoende compliantie op te leveren, werd positief geëvalueerd en maakte het fluctuerende verloop van klachten gedurende de dag inzichtelijk, op basis waarvan geconcludeerd is dat het een haalbare en geschikte methode is bij patiënten met PDS. Catastroferende cognities bleken zowel als antecedent en als consequentie van buikpijn op te treden, terwijl geen relaties tussen stress en buikpijn werd gevonden. De implicaties van deze bevindingen worden bediscussieerd. Engelstalige samenvatting Irritable Bowel Syndrome (IBS) is estimated to occur in 10-20% of the general population. Symptoms are abdominal pain, disturbed defecation and other gastrointestinal complaints, like bloating, flatulence and abdominal rumbling. To date no somatic explanation for IBS has been identified. It has been suggested that psychosocial factors (e.g. catastrophizing cognitions, stress) play a role in precipitating and perpetuating IBS-symptoms. Symptoms wax and wane as time passes and even during the day. However, there’s a lack of insight in these fluctuations and how they relate to other factors. Electronic diaries overcome some important drawbacks of paper questionnaires and capture fluctuating psychological states and physical symptoms by measuring phenomenons in their natural environment at the moment they occur. IBS-patients completed electronic diaries three or four times daily for several weeks. According to the results, compliance is acceptable, the method is positively evaluated by patients and captures fluctuations of symptoms during the day, leading to the conclusion that the method is feasible and well suitable for this patient population. Catastrophizing cognitions were found as antecedents as well as consequences of abdominal pain, whereas no relations between stress and abdominal pain were found. Implications of these results are discussed.
